Health Care for the Homeless (HCH) provides comprehensive primary care and behavioral health services throughout the Queen City to unhoused people, including those experiencing homelessness, living in treatment centers, and staying with friends. Resources include Medication Assisted Therapy (MAT) for substance use and connection to social services.
All people who are currently experiencing homelessness, or who are at risk of being homeless, are welcome. Payment is determined on a sliding scale, but no one is turned away due to an inability to pay.
The HCH clinic team includes physicians, nurse practitioners, physician assistants, nurses, medical assistants, therapists, transition of care coordinators, and support staff. HCH collaborates with social service agencies and providers to improve the overall health of those served.
